P.O.M Plan (Peace of Mind)
A ‘P.OM’ (Peace of Mind) Plan is a document where you can write down everything about the person you care for and their needs and preferences as well as helping you capture the most important information about your caring role all in one place. Your POM plan can support you to:
- Focus on what matters most to you and your family
- Set small, achievable goals for your wellbeing and caring role
- Record key contacts and steps for emergencies
- Find a balance between caring and looking after yourself
- Share important information quickly with professionals or loved ones
By creating your own POM plan, you’ll have a clear and practical guide to support you day-to-day and give you peace of mind for the future. It’s essentially a personal roadmap that keeps the Carer’s needs and the cared-for person’s wellbeing in focus, while making sure support is easy to access when required.
Workshops will be delivered across the community to help Carers create and develop their own P.O.M (Peace of Mind) plan. These sessions are designed to give Carers the tools and guidance they need to identify their priorities, set goals, and plan ahead with confidence.
